The Hospital Director’s Office is situated in the south-eastern corner of the fourth floor. A large desk takes center stage along the back wall, with two armchairs facing it, and a low couch and coffee table sit along the left hand wall as you come in the door. There is a private bathroom off the office that houses a shower, and a wardrobe for the Director’s use only. The Director’s Assistant’s desk is situated in a receiving room just before the Director’s office. When the door is shut the office is completely sound proof for reasons of confidentiality, and an intercom on the desk connects it to the Assistant out front.

The Janus Thickey Ward is located on the fourth floor, and is usually kept locked to prevent patients from wandering around. It houses patients with permanent or long-lasting spell damage. 'Famous' residents include Alice and Frank Longbottom, and Gilderoy Lockhart.

The fourth floor is where one would go if they had any spell damage, from any number of hexes, jinxes, or poorly placed charms. For anyone with long-term spell damage, there is the Janus Thickey Ward, which is usually locked to keep patients from wandering around. The hospital director's office is located here as well.
